To make every visit convenient and comfortable, here’s everything you need to know about our Fort Myers office — from what to bring to your first visit to our hours, location, insurance, and how to schedule. As Healthworx Active, we’re the same trusted team you knew as Footworx Active, with the same commitment to caring for the whole person, every visit.
New Guests Welcome
Before your first visit, we strive to gather important background information, like your medical history, so you can spend your appointment getting to know your doctor.
Please arrive 10 minutes early so we can complete your initial paperwork. Also, please bring the following:
- Guest’s ID and insurance card
- A list of current prescriptions and/or over-the-counter medications, including dose and frequency
- Information about the guest’s medical and surgical history
- Recent test results, x-rays, or relevant records
If for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before the scheduled appointment time.
Insurance & Payment
You are responsible for any co-payments or charges not covered by your insurance. Questions regarding insurance coverage and benefits should be directed to your insurance company. Please call your insurance carrier to confirm whether Dr. Sadri is in your network.
